    The Annual Physical Reward is a $50 monthly discount on your medical rates. Participating in the Annual Physical Reward is completely voluntary. Choosing not to participate will only result in not receiving the Reward.

If you and your covered family members have been tobacco-free for at least 30 days, or have never used tobacco, you will pay $100 less per month for your medical coverage. You will be asked to declare your tobacco status during your annual enrollment session. If you declare that you or any of your covered family members are not tobacco-free, you will have a $100 per month pre-tax tobacco surcharge applied to your paycheck.
