Benefits Enrollment Checklist

Take time to learn about and understand your benefits before enrolling. Use this checklist to help you prepare.

• Enroll or waive coverage during your enrollment period.

• Have all of your dependents’ Social Security numbers available as they are required.

• Verify dependents newly added to medical coverage on WellatNewell.com within 30 days of your enrollment event.

• If you elect the Annual Physical Reward, you must provide medical information for yourself (and your spouse/partner if covering their medical coverage). You will certify during your enrollment period that you and your spouse/partner have completed an annual physical in the plan year, or within 90 days as a new hire.
